About Supporting Smooth Transitions

Some children may need particular help, including from other agencies, to ensure that their transition is as smooth as possible. 

Research suggests that transitions are central to young children’s development and emotional wellbeing. How the first transitions are handled can have a significant impact on a child’s capacity to cope with change in the short and long term and impact on the progress they make from their starting points. 

The Supporting Smooth Transitions toolkit is a suite of resources created by professionals working within the health and education that can be used to supplement current transition procedures within schools and settings.

The Transition Level of Need tool (TLoNt) has been developed for schools and settings in Hertfordshire to share information about any potential needs to enable effective transition to be implemented for every child.

Activity cards (digital versions) 

Our activity cards have been developed for children starting Reception as part of the Supporting Smooth Transition procedures.  Parents and carers can keep them handy on their digital devices for when they are doing things at home or are out and about, to help their child find out about new things and develop the skills that will help learning at school.

The poster can be printed and displayed in schools or settings so that parents/carers can access the digital version or the cards can be downloaded and sent digitally to families.
